Online Application
We can provide some funding for all re-entry needs; transportation, clothing, food, housing, identification documents and job placement.
We have an application process which must be complete before services can be provided. To obtain an application, contact us at rick@chainsofgrace.org or via our hard mail address, PO Box 1344 Midlothian, Texas 76065. You can also call Rick Davis at 972-740-1917, or fill in the applications below.
No persons will be approved for help through COG without a completed, approved application. Please follow and complete the type of application that best fit your needs below.
Benevolence Fund Request
We understand that everyone could use a little help from time to time. The Chains of Grace Benevolence Fund is intended to help provide one-time support for an individual or family facing circumstances where some additional financial support is required. If you would like to request Benevolence Fund support, please complete the request form online.
Housing Request
Chains of Grace provides transitional housing for those recently paroled. If you or a loved one would like to apply for transitional housing, please fill out the form below with a message describing your situation and we will get back to you.
